An outcome evidence register helps consultants trace each results claim to a source, period, owner, and permission status.
Consultants often discover useful results in scattered places: a dashboard screenshot, an email from the sponsor, a process record, or a comment during a follow-up call. Months later, nobody can confirm the source or whether the client approved public use.
An outcome evidence register keeps those facts traceable.
Give every claim a source
Create one row for each result the client may need to review:

Link to the original record instead of copying a number into several decks. That reduces the chance that different versions circulate.
Separate observation from attribution
“Cycle time fell from 12 days to 9” is an observation. “Our workshop reduced cycle time by 25%” is an attribution claim. The second statement needs stronger evidence because other changes may have contributed.
The Magenta Book explains that evaluation considers what would have happened without an intervention and examines unintended effects. Use that caution when describing client outcomes. State what the evidence shows and what remains uncertain.
Preserve qualitative evidence carefully
A client quote can explain why a change mattered, but it should include the speaker, date, and context. Record whether the wording is exact or summarized.
Never treat a private message as permission to publish. Ask the client to approve the final wording, attribution, and placement before using it in a testimonial or case study.
Review the register at closeout
Confirm that each important outcome has:
- an agreed baseline
- a named source
- a client validator
- a future review date when evidence is still developing
- an explicit permission status
Archive the register with the project handover package. Carry open measures into the benefits tracker.
Make credible results easy to find
An evidence register is not a marketing claims folder. It is a decision record. It helps the client inspect progress and helps you avoid exaggerating what the engagement achieved.
Start with one outcome. Link the source, record the caveat, and keep public-use permission separate from internal validation.
